The City provides the following benefits to eligible full-time and half-time employees through its LAwell Civilian Employee Benefits Program (“LAwell”).
Medical Insurance Employees may choose from five different plans: (1) Kaiser Permanente (HMO) (2) Anthem Narrow Network (Select) HMO (3) Anthem Full Network (CA Care) HMO (4) Anthem Vivity (LA & Orange Counties) HMO
(5) Anthem Preferred Provider Organization (PPO)
The HMO options provide healthcare through networks of doctors, hospitals, and healthcare providers, with member care managed by a Primary Care Physician; to enroll, a member must reside/work within the region served by the HMO. The PPO option provides access to networks of doctors, hospitals, and other healthcare providers, both regionally and nationally; members can access services both within and outside of a network, with the latter generally being at higher cost to the member.
Vision Insurance Employees receive vision care benefits through a single vision plan, offered through Blue View Vision Plus.
Dental Insurance Employees may choose from three different plans: (1) DeltaCare USA HMO (2) Delta Dental PPO (3) Delta Dental Preventive Only
The HMO option provides dental care through a network of dentists; the PPO option makes dental care available both inside and outside of a dental care network; the preventive-only option covers only preventive services.
Life Insurance LAwell provides a base life insurance policy through The Standard Insurance Company; employees also have the option to purchase higher levels of coverage.
Disability Insurance LAwell provides a base policy through The Standard Insurance Company; employees also have the option to purchase higher levels of coverage.
Accidental Death and Dismemberment LAwell provides the option to purchase this coverage through The Standard Insurance Company.
Healthcare Flexible Spending and Dependent Care Reimbursement Accounts LAwell provides a tax-savings option to set aside money for eligible healthcare and day care expenses. Funds set aside in these accounts must be used in the calendar year and do not roll over to future years. Accounts are managed through WageWorks.
Support Plus LAwell provides an Employee and Family Assistance Program (EFAP) offering counseling services at no cost to the employee. Optum is the EFAP provider.
