Join us on the fourth Thursday of each month from March to September 2018 as the Employee Benefits Division begins its benefits lunchtime seminars for 2018. These seminars are designed to cover educational topics to help you better understand how to use your LAwell benefits and will also provide you the opportunity to ask questions of the City's benefits providers and Benefits Division staff.
Form 1095-C will be mailed to your home address on file no later than March 2, 2018. Per the Internal Revenue Service, you are not required to attach Form 1095-C to your tax return. What is a Medical History Statement?
Employees who enroll in specific levels of Life or Disability insurance may be required to complete a Medical History Statement with the Standard Insurance Company before their elected insurance level can take effect.
